Heparin: (txt.1451) is an anticoagulant, and it suppress clot formation. Because of the action of this medication the nurse must continually assess the client for signs of bleeding, such as increased bruising, guiac-positive stools, and bleeding gums. ALSO, watch for concurrent use of other thrombolytic agents or med’s that affect platelet function, or cause hypoprothrombinemia

as for #1. Describe the 3 levels of care : primary, secondary, tertiary - Txt. 365 didn't really do it for me. I think the ATI study guide definition might be what FS is looking for here.
ATI p.1
Primary Care: Promotion, prevention, early detection (ie: testing for diabetes)
Secondary Care: Diagnosis and treatment, acute care, emergency care (ie: diagnosing/treating diabetes)
Tertiary Care: Long-term care, rehab (ie: managing diabetes)
